X-ray

The X-Ray Safety Program establishes a set of requirements for those working with x-ray devices at the University of Waterloo.

Training

All x-ray users require x-ray safety training in order to be added to the x-ray permit. X-ray safety training is a three-step process:

  1. Complete X-ray Safety (SO1011) including review of the content and the quiz.
  2. Complete one-on-one practical training provided by the x-ray permit holder or competent designate (see X-Ray Safety Program for definition). This training must be documented in the lab. Documentation is up to the lab. Some options include:
    1. Detailing practical training on the equipment SOP and signing off at the end of the SOP
    2. Completing a site-specific training form
  3. In X-ray Safety Practical (SO1011-P), complete the "Permit Specific Questions" section (click on Submit and then select Quizzes). You will need your permit number, permit holder name, and details of the practical training to submit this. The XSO will review the submission in SO1011-P and contact the individual and the permit holder when the individual has been added to the permit. If the answers provided in the "Permit Specific Questions" section are insufficient, the individual will be required to resubmit it prior to being added to the permit.

Once you receive confirmation from the XSO that you have been added to the permit, you may start using the x-ray device.
